User IP showing server IP

I noticed that users in the user online section are showing the server IP as their IP? It doesn't show the users real IP. I'm using cloudflare but I've turned off the cloudflare proxy but it still shows the server IP. What's going on here?

SoftCreatR I'm already using version 5.4 of this plugin and nothing changed. It's been a few months ago when I installed it.

Possibly you are using a reverse proxy, for example, Apache behind nginx, and it is not properly configured to restore the visitor IP address. It is strongly recommended to fix the web server configuration, then you won’t need any plugin too.

I am using nginx-varnish-apache. I guess I need find out how to configure it to restore the IP.

Whats strange is a few users are showing real IPs but 90+% are showing the server IP.
