Actual applications for moderated user groups!

I love Moderated user groups idea. However, in practice their reality is pretty worthless.

I wish to request a plugin that allows a form builder in the moderated user group apply. Reason, with no where to even say what reason needs to have in it, is pretty worthless.

In my use case, I need to ask the users questions, before I just let them in. Honestly not sure why else even have the ability to "Apply".
